Samantha Gail Ortiz was born on September 10, 1990, in Louisville, Kentucky, and passed away on July 21, 2026. Her presence touched many lives, leaving cherished memories and a legacy of love, creativity, and devotion that will never be forgotten.

Samantha’s greatest pride and the most important title she ever embraced was that of Mother. Her children—her daughter Amelya Douglas, and her sons Alejandro, Alexander, and Miguel "Guelly" Ortiz—were the very center of her universe. She poured her unwavering love, care, and support into their lives, carefully nurturing them and striving to capture every cherished moment together. Samantha was known for her admiration and dedication, constantly taking photos and videos, always wanting to preserve the sweet memories she created with her beloved children.

From an early age, Samantha shared a close bond with her grandparents, Lee and Anna Sue Douglas. Her grandfather bestowed upon her the fond nickname “Catfish,” a testament to their special relationship—a memory that brought warmth to her heart throughout her life.

In her role as daughter, Samantha was deeply loving and loyal. She is survived by her mothers, Angela Douglas, and Adrian Novak and her father Patrick Horn and his spouse Missy. As a sister and friend, Samantha was steadfast and true. Her relationship with her brother Aaron—fondly known as Digger—was unique and inseparable; they were best friends who shared many adventures and confidences.

Creativity flowed naturally to Samantha. She was artsy with an unmistakable flair for home décor, often dedicating her time to painting and creating unique, personalized pieces for her home and family. Her artistic expressions added a distinct touch to every space she inhabited. Music also played an integral part in Samantha’s life, providing a rich soundtrack that matched her vibrant spirit and appreciation for all genres and styles.

Animals, especially cats, held a special place in Samantha’s heart, and her fondness for owls was well-known among those closest to her.

Samantha’s extended family includes her beloved sister-in-law Tiffany Horn, aunts Rosie Sprinkles, Laura Coomes, Adja Diop Douglas, and her uncles James, Christopher Douglas, and Donald. She is lovingly remembered by her niece Ava Presley, her nephews Logan Horn and Easton Horn, and a host of cousins: John Richardson, Jacob Richardson, Paul Ray, Toni, DJ, Dalton, Adam, and Joshua.

She is preceded in death by her beloved grandfather Lee, her grandmother Anna Sue Douglas, her aunt Rebecca Richardson, and her cousin Joshua Sprinkles. Their memories live on in the hearts of all who knew Samantha.

Those who were privileged to know Samantha will remember her kindness, her unwavering support for her children, her dedication to family, her creativity, and the joy she found in the simple pleasures of life.
